Two distinct physiological processes are relevant to a brief cold-water plunge, and understanding the difference between them explains why the activity is considered manageable within a short immersion window despite how extreme it sounds.
Cold shock is the body's immediate reaction to sudden cold-water contact: an involuntary gasp reflex, rapid breathing, and a spike in heart rate and blood pressure. Effects typically peak around 30 seconds after entry, and the acute response can continue for one to three minutes. This is the window during which supervision and a calm, controlled entry matter most, which is why expedition crews structure the plunge around a fast entry and an immediate, supervised exit rather than an extended swim.
Hypothermia is a separate, longer-developing process and is not considered a significant risk during a plunge lasting 10 to 15 seconds. It generally becomes a serious concern only after prolonged exposure of 30 minutes or more, depending on water temperature and individual factors, well beyond the duration of a standard polar plunge. The brevity of the activity is precisely why it is structured the way it is: a quick entry, a quick exit, and an immediate warming protocol.
Before the plunge, most operators build in a medical check-in with the ship's doctor or medical officer, either as a standing part of the waiver process or as a specific conversation for travelers with relevant health conditions. Conditions commonly flagged for a doctor's input include cardiac conditions, respiratory conditions such as asthma, pregnancy, and Raynaud's disease or similar cold-sensitivity conditions.
Common on-site measures include a medical officer present or on standby, warm towels and robes staged at the exit point, immediate access to a warm interior space, and rescue crew or a Zodiac positioned near the entry point. Safety tether or harness use is reported at some operators but is not confirmed as a universal standard across the industry. The activity is also typically suspended outright if weather or sea conditions deteriorate.
